For opioid use disorder, the MAT formulary at Charlotte includes Suboxone (buprenorphine/naloxone) as the primary agent, plus Vivitrol (extended-release naltrexone) and Sublocade (monthly buprenorphine injection). Methadone is not in our formulary; clients who need methadone are referred to a federally licensed opioid treatment program in the Charlotte metro.
For non-opioid substance use disorders, behavioral interventions are primary. CBT targets the trigger-thought-craving chain; DBT supports emotion regulation; motivational interviewing handles ambivalence; relapse-prevention planning maps substance-specific triggers. ASAM Criteria (substance use), LOCUS (mental health acuity), PHQ-9 (depression), GAD-7 (anxiety), Columbia Suicide Severity Rating Scale, biopsychosocial history, nutrition, and pain screens. Co-occurring mental health conditions are part of the treatment plan from day one.
Family therapy and family support work, with the client's signed release, run alongside primary treatment.
Trauma-informed practice is the default across groups and individual sessions. EMDR is available for clients who screen in. We use "EMDR available" and "trauma-informed care" because that is what we deliver.
