The goal is the same as our medical detox coordination model: remove friction, preserve clinical integrity, and ensure the handoff into outpatient step-down is seamless, not fractured.
1. First call. A clinical phone assessment plus insurance verification is completed during the same call at (888) 464-2144. 2. Concierge placement. If inpatient is clinically indicated, the admissions team identifies an accredited partner facility matched to the client's clinical needs, insurance, and (if relevant) any geographic or program preferences. 3. Logistics coordination. Transportation, admission paperwork, insurance authorization, and family communication (with releases) are handled. 4. Continuity during inpatient stay. A point of contact at Archangel stays in light contact with the client and family during the stay, with appropriate releases. 5. Step-down to outpatient. Discharge planning begins well before the inpatient discharge date. The intake into our outpatient program (typically Partial Care) is scheduled before the inpatient stay ends, so there is no unsupervised gap.
Why the coordinated handoff matters
The 24 to 72 hours after discharge from inpatient care is the single highest-risk window in early recovery. Clients leave a structured environment and re-enter the environment that contributed to the addiction in the first place. The plan that exists or does not exist at that moment determines what happens next.

After inpatient, what step-down looks like
